Mail Form 3911 to the IRS Refund Inquiry Unit that covers your state, not to the IRS address printed at the top of the form. The IRS runs eight regional Refund Inquiry Units, and each one handles a specific set of states. Send it to the wrong unit and it will sit before being rerouted. You can also fax the form to the same unit, which is usually faster than mail.1Internal Revenue Service. About Form 3911, Taxpayer Statement Regarding Refund
The form itself warns against using the header address, which is just the national office.2Internal Revenue Service. Check Whether You Actually Need to Mail Form 3911
Before you drop the form in the mail, confirm it is the right step. Some of these situations resolve without ever filing 3911.
Try the Automated Trace First
You can start a refund trace without Form 3911 through the “Where’s My Refund?” tool on irs.gov, the automated line at 800-829-1954, or by calling 800-829-1040 to speak with a representative. Joint filers are the exception: the automated systems will not open a trace on a joint return, so joint filers need to either call a representative or file Form 3911.3Internal Revenue Service. Make Sure Enough Time Has Passed
The IRS typically issues refunds within 21 days of accepting an e-filed return.4Internal Revenue Service. Why It May Take Longer Than 21 Days for Some Taxpayers to Receive Their Federal Refund Paper returns take six weeks or more.5Internal Revenue Service. Let Us Help You Inside those windows, the refund likely has not been issued yet, and Form 3911 will not speed anything up. For a lost paper check, “Where’s My Refund?” lets you file an online claim for a replacement once it has been more than 28 days since the check was mailed.6Internal Revenue Service. Rule Out an Offset
If you owe back taxes, past-due child support, or certain other federal or state debts, the Treasury Department’s Bureau of the Fiscal Service can intercept your refund and apply it to the debt. When that happens the IRS sends a CP49 notice explaining what was taken.7Internal Revenue Service. Wrong Bank Account? Start With the Bank
If a direct deposit went to the wrong bank because you entered an incorrect routing or account number, contact the financial institution first. If five calendar days pass with no resolution, then file Form 3911 to have the IRS step in.9Internal Revenue Service. Refund Inquiries
Tips for Sending It
Send Form 3911 by certified mail with return receipt requested. That gives you a verifiable delivery date if there is ever a dispute about whether the IRS received it. Faxing also creates a timestamp and skips postal delays entirely.
Keep a copy of everything you send. Pull up your original tax return before you fill out the form, and copy the refund amount, filing date, and bank information exactly. Small discrepancies between your Form 3911 and your original return slow the trace down, because the IRS has to reconcile the mismatch before moving forward.
If you filed jointly, both spouses have to sign before the IRS will begin the trace. Line 11 is set aside for the spouse’s signature, and a missing second signature is one of the most common reasons these forms come back.2Internal Revenue Service. Once the form is in, the typical trace takes about six weeks.10Taxpayer Advocate Service. Lost or Stolen Refund If that window passes without any word from the IRS, call 844-545-5640 to schedule an appointment at a Taxpayer Assistance Center.5Internal Revenue Service. Let Us Help You
