What Is the Payer Name on Form 1099-SA? Custodian Role and Fixes

The payer name on Form 1099-SA is the financial institution that holds your Health Savings Account, Archer MSA, or Medicare Advantage MSA — the trustee or custodian of the account, typically a bank, brokerage firm, or insurance company. It is not your employer, even if your employer chose the custodian for you during benefits enrollment.

Where the Payer Appears on the Form

The payer’s name, address, and Taxpayer Identification Number sit in the upper-left corner of Form 1099-SA. If you opened your HSA through Fidelity, Fidelity is the payer. If your employer selected HealthEquity or Optum Bank as the plan’s custodian, that custodian is the payer. Your employer’s name does not belong in that box and won’t appear there.

The IRS uses the payer’s TIN to match what shows up on your form against the custodian’s own reporting. That’s why the name has to be the institution actually holding the money, not the company that set up the benefit.

Why It’s the Custodian and Not Your Employer

Employers can sponsor an HSA program and even contribute to it, but they don’t hold the funds. The account belongs to you, and it lives at whichever financial institution acts as trustee. That trustee is the party responsible for tracking every withdrawal and reporting it to both you and the IRS, regardless of the dollar amount. There is no minimum threshold. If you’re not sure which institution that is, check your account statements, your online HSA portal, or the debit card tied to the account. The name on those documents is the name that should appear as the payer.

What Else the Payer Fills In

Once you’ve confirmed the payer is the right institution, the rest of the form tells you what that institution reported about your account:

  • Box 1 shows the gross distribution — the total amount withdrawn during the tax year, including any payments the trustee sent directly to a medical provider on your behalf.
  • Box 2 shows earnings on excess contributions you pulled back out before the tax deadline. That figure is already included in Box 1.
  • Box 3 carries a single-digit distribution code explaining why the money left the account.

Most account holders see Code 1 in Box 3, the default for a standard withdrawal. The custodian doesn’t know how you spent the money, so Code 1 doesn’t say anything about whether the distribution is taxable. You sort that out on Form 8889 when you file.

When the Payer Must Send It

The custodian must furnish your copy of Form 1099-SA by January 31 following the year of the distribution.2Internal Revenue Service. 2026 Publication 1099 If you took HSA withdrawals in 2025, expect your form by the end of January 2026.

If it hasn’t shown up by mid-February, contact the trustee and ask for a copy. If you still don’t have one by the end of February, call the IRS at 800-829-1040. If the Payer Name Is Wrong

Errors happen. If the payer name, address, or TIN on your 1099-SA doesn’t match the institution that actually holds your account — or if the distribution amount or code is off — call the trustee first and request a corrected form. If the custodian won’t cooperate, call the IRS at the number above. Provide the payer’s name, address, and phone number along with your own identifying information, and the IRS will contact the payer. File your return using the figures you know to be correct from your own records, and keep documentation in case there’s follow-up.

One Situation That Won’t Produce a Form

If you moved your HSA from one custodian to another through a direct trustee-to-trustee transfer, no 1099-SA is issued. A 60-day rollover, where the old custodian actually cut you a check, is different — that one does generate a 1099-SA with Code 1 in Box 3, and you reconcile it on Form 8889.
