What Is a Tax Payment Voucher: Forms, Mailing, and Deadlines

A tax payment voucher is a paper slip you mail to the IRS with a check or money order so the agency can match your payment to the correct taxpayer, tax form, and tax year. You only need one if you are paying a federal tax bill by mail; electronic payments carry the same identifying information automatically, so no voucher is required. The right voucher depends on what you owe: individual income tax, quarterly estimated tax, employment tax, fiduciary tax on an estate or trust, or an extension payment each has its own form.

Without a voucher, the check that arrives at an IRS processing center is just a check. Staff may not be able to connect it to your return, which can produce a balance-due notice and late-payment penalties for a bill you actually paid.

Which Voucher You Need

Five federal forms cover almost every situation where a voucher comes into play.

Form 1040-V goes with a balance-due payment on your annual individual return. It works for Form 1040, Form 1040-SR (age 65 and older), and Form 1040-NR (nonresident aliens).1Internal Revenue Service. Form 1040-V Payment Voucher for Individuals Skip it if you owe nothing or you pay electronically.

Form 1040-ES is for quarterly estimated tax. If you have self-employment earnings, freelance income, dividends, or rental income without withholding, you generally pay estimated tax four times a year. The form includes four detachable vouchers, each pre-labeled for its quarter so the IRS credits the payment to the right period.2Internal Revenue Service. Form 941-V accompanies the employer quarterly return, Form 941. Its use is narrow: you can pay by check with this voucher only if total quarterly liability is under $2,500 and you did not trigger a next-day deposit obligation during the quarter.3Internal Revenue Service. Form 1041-V is the fiduciary equivalent of 1040-V. Estates and trusts that owe a balance on Form 1041 attach it to the check.4Internal Revenue Service. About Form 1041-V, Payment Voucher

Form 4868 is different. When you file for an automatic extension of time to file your individual return, Form 4868 itself acts as the payment voucher. There is no separate slip.5Internal Revenue Service. If you expect to owe, send payment by the original April deadline anyway.

Filling It Out Without Causing a Problem

Every voucher asks for the same core information. On Form 1040-V, which is representative of the rest, you enter:1Internal Revenue Service. Form 1040-V Payment Voucher for Individuals

  • The Social Security number shown first on your return, plus your spouse’s SSN on the second line for a joint return. Businesses use their Employer Identification Number instead.
  • The payment amount, in dollars and cents, matching the check exactly. Use the format $XXX.XX with no dashes or lines.
  • Your name and address, printed exactly as they appear on your return.
  • The tax period. For annual returns this is the tax year, such as “2025.” Estimated tax vouchers are already labeled with their quarter.

The tax period is the field that causes the most trouble when it is wrong. Write the wrong year and the IRS may credit your money to a year you do not owe for, then send you a balance-due notice for the year you actually meant to pay. Fixing that takes time and phone calls.

Preparing and Mailing the Check

Make the check or money order payable to “United States Treasury.” On the check itself, write your name, address, daytime phone number, SSN or EIN, the tax year, and the form number, for example “2025 Form 1040.”6Internal Revenue Service. Joint filers who received separate balance-due assessments should also write “MFT 31 separate assessment” on the check.

Place the voucher, the check, and your return loosely in the envelope. Do not staple or paper-clip anything. IRS processing centers run mail through high-speed scanners, and fasteners can jam the equipment or damage your documents.6Internal Revenue Service. Pay by Check or Money Order

Use regular USPS mail when sending Form 1040-V with your return. The IRS specifically warns that using private delivery services for 1040-V payments may delay processing.6Internal Revenue Service. Pay by Check or Money Order The mailing address is not universal; it depends on the form and the state you live in, so check the instructions that came with your specific form for the right service center.

Making the Deadline Count

For tax year 2025, the individual income tax filing deadline is April 15, 2026.7Internal Revenue Service. IRS Opens 2026 Filing Season Estimated tax payments are due on the 15th day of the 4th, 6th, and 9th months of the tax year, and the 15th of the 1st month after the tax year ends.8Internal Revenue Service. Publication 509 (2026), Tax Calendars For calendar-year taxpayers, that is April 15, June 15, September 15, and January 15 of the following year. When a due date falls on a weekend or holiday, it shifts to the next business day.

Federal law treats a timely postmark as timely payment. Under 26 U.S.C. ยง 7502, a payment mailed with a USPS postmark on or before the due date counts as paid on that postmark date, even if the envelope arrives later.9Office of the Law Revision Counsel. 26 USC 7502 – Timely Mailing Treated as Timely Filing and Paying The protection extends to certain designated services from DHL, FedEx, and UPS, but only specific service levels qualify.10Internal Revenue Service. One risk to know about: the USPS postmark reflects the date your mail is processed at a regional facility, not the moment you handed it to a clerk. Drop a payment off on April 15 in the afternoon and it may not be postmarked until April 16. Mailing a day or two early avoids the problem.

Get Proof You Mailed on Time

If you are mailing close to the deadline, use USPS Certified Mail with a return receipt. You get a postmarked receipt showing the mailing date, a tracking number, and signed confirmation when the IRS receives the envelope. It runs roughly $9 to $11. Regular first-class mail offers no proof of when you sent it, and if the IRS says the payment was late, the burden is on you to show otherwise.

If the Payment Is Late or Misapplied

The failure-to-pay penalty is 0.5% of the unpaid tax for each month or partial month the balance remains, up to 25%.11Internal Revenue Service. If the IRS credits your payment to the wrong year or account, you will typically receive a CP60 notice explaining the transfer and showing a balance due.13Internal Revenue Service. Understanding Your CP60 Notice If the transfer was wrong, mail proof of payment (a canceled check image or your certified mail receipt) to the address on the notice before the deadline it gives. Ignoring it lets interest and penalties keep growing on the balance the IRS says you owe.

Skip the Voucher by Paying Electronically

Paper vouchers exist because a physical check needs identifying data attached. Every electronic method sends that data automatically, so no voucher is needed. Electronic payments are also faster, generate an instant confirmation, and cannot get lost in the mail.

IRS Direct Pay. Schedules a payment from your checking or savings account through the IRS website or the IRS2Go mobile app at no cost. EFTPS. The Electronic Federal Tax Payment System is the government’s dedicated platform for scheduled tax payments. It is common for business payroll deposits, and individuals can use it too. Enrollment takes about a week because the IRS mails you a PIN. Once enrolled, you can schedule payments up to 365 days ahead.

Card or digital wallet. The IRS accepts payment through approved third-party processors by credit card, debit card, or digital wallets like PayPal and Venmo. Debit card payments cost roughly $2.10 to $2.15 per transaction; credit card payments run 1.75% to 1.85% of the payment amount. All fees go to the processor, not the IRS.16Internal Revenue Service. E-file with electronic funds withdrawal. Most tax software lets you authorize a bank debit when you e-file. You enter your routing and account number, pick a payment date up to the filing deadline, and the software transmits everything together. For most individual taxpayers this is the lowest-friction option because there is no separate step after filing.
