Kaiser Permanente members generally receive between one and three tax documents a year. Which of the Kaiser tax forms show up in your mailbox or member portal depends on how you enrolled in your plan and whether you have a Health Savings Account. Most people get a single coverage form for their records. The one that actually affects your refund or balance due is Form 1095-A, and only if you bought your Kaiser plan through a Health Insurance Marketplace.
Which Forms You Get and Who Sends Them
Five forms are in play, and they come from three different places.1Kaiser Permanente. Understanding Your IRS 1095 Forms
- Form 1095-A comes from the Health Insurance Marketplace, not Kaiser, if you enrolled through a state or federal exchange.
- Form 1095-B comes from Kaiser Permanente if you have an individual, family, or small-employer plan.
- Form 1095-C comes from your employer if the company has 50 or more full-time employees.
- Form 1099-SA comes from your HSA custodian (typically Healthcare Bank for Kaiser members) if you took money out of your HSA during the year.
- Form 5498-SA also comes from your HSA custodian, reporting contributions and account balance. It arrives later, often in May.
None of these forms get attached to your return.2Internal Revenue Service. Questions and Answers About Health Care Information Forms for Individuals You keep them with your records. But you’ll need the numbers from Form 1095-A and Form 1099-SA to complete specific IRS worksheets, so don’t lose them.
Form 1095-A: The One That Changes Your Tax Bill
If you or a family member enrolled in a Kaiser plan through HealthCare.gov or a state exchange, watch for Form 1095-A from the Marketplace by January 31.3Internal Revenue Service. About Form 1095-A, Health Insurance Marketplace Statement You retrieve it by logging into the same exchange account you used to enroll. Kaiser does not issue it and cannot provide a copy.
Part III of the form is where the important numbers live, reported month by month across three columns:4Internal Revenue Service. Instructions for Form 1095-A
- Column A: your monthly enrollment premium.
- Column B: the premium for the benchmark plan (the second-lowest cost Silver plan in your area). This drives your maximum credit.
- Column C: the advance premium tax credit paid to your insurer on your behalf each month.
Before you start your return, check these figures against your own records. An error in any column will throw off your credit calculation.
Reconciling on Form 8962
If any advance premium tax credit was paid during the year, you must file IRS Form 8962 with your return.5Internal Revenue Service. Instructions for Form 8962 Form 8962 compares what the government sent to your insurer against the credit you actually qualify for based on your final income. You’ll either claim additional credit as part of your refund or repay excess credit you weren’t entitled to.
This is not optional. If you file electronically and leave out Form 8962, the IRS will reject the return under business rule F8962-070.6Internal Revenue Service. How to Correct an Electronically Filed Return Rejected for a Missing Form 8962 The rejection message will tell you the IRS has records showing advance credits were paid on your behalf, and you can’t complete filing until you include the form.
Repaying Excess Credit
When your actual income comes in higher than what you estimated on your Marketplace application, the credit you were entitled to is smaller than what was paid, and you owe back the difference. For tax year 2025 returns filed in 2026, the amount you have to repay is capped based on how your household income compares to the federal poverty line, with lower-income filers repaying less and filers at 400% of the poverty line or above owing the full excess.7Internal Revenue Service. 2025 Instructions for Form 8962
That cushion is going away. For tax year 2026 and beyond, the repayment caps are eliminated entirely.8Internal Revenue Service. Updates to Questions and Answers About the Premium Tax Credit Starting with returns filed in 2027, filers at every income level will owe back the full excess with no dollar limit. If your income changes during the year, updating your Marketplace application quickly matters more than it used to.
Form 1095-B: Your Proof of Coverage
Kaiser sends Form 1095-B directly when you bought individual or family coverage outside the Marketplace, or when your small employer’s fully insured plan runs through Kaiser.1Kaiser Permanente. The federal penalty for going uninsured dropped to $0 starting in 2019, so this form no longer affects your federal return.10Internal Revenue Service. Questions and Answers on the Individual Shared Responsibility Provision It still matters if you live in California, Massachusetts, New Jersey, Rhode Island, or the District of Columbia, all of which enforce their own coverage mandates with real financial penalties. The 1095-B is your proof of compliance. Even if your state has no mandate, keep it in case the IRS or a state agency questions your coverage status.
Form 1095-C: Coverage From a Large Employer
Employers with 50 or more full-time employees are classified as Applicable Large Employers and must send Form 1095-C.11Internal Revenue Service. About Form 1095-C, Employer-Provided Health Insurance Offer and Coverage Even when Kaiser is the insurer behind your plan, the employer produces this form because it reports whether coverage was offered, whether it met affordability standards, and which months you were enrolled.
For plan years beginning in 2026, employer-sponsored coverage counts as affordable when your share of the premium for self-only coverage doesn’t exceed 9.96% of your household income. If your employer’s plan costs more than that or doesn’t provide minimum value, you could qualify for Marketplace subsidies instead. That determination shows up in codes on the 1095-C.
Form 1099-SA: Money You Took Out of Your HSA
Kaiser members with a High Deductible Health Plan who use a Health Savings Account will get Form 1099-SA reporting every withdrawal made during the year.12Internal Revenue Service. About Form 1099-SA For most Kaiser HSA holders, it comes from Healthcare Bank, which serves as the account custodian.13Kaiser Permanente. Health Savings Account (HSA) Box 1 shows total distributions; Box 3 contains a code indicating the type of withdrawal.
You use the numbers on Form 1099-SA to complete IRS Form 8889, which reports your full HSA activity for the year including both contributions and distributions.14Internal Revenue Service. Form 8889 – Health Savings Accounts (HSAs) The IRS does not automatically know whether the money went toward medical bills or something else. You’re responsible for tracking that and reporting it correctly.
What the Tax Treatment Depends On
Distributions spent on qualified medical expenses are tax-free. The IRS defines qualified expenses broadly in Publication 502 and includes doctor visits, prescriptions, dental work, vision care, and mental health treatment.15Internal Revenue Service. Publication 502 – Medical and Dental Expenses
Money pulled out for anything else gets added to your taxable income and hit with an additional 20% penalty tax, calculated on Form 8889.16Office of the Law Revision Counsel. 26 USC 223 – Health Savings Accounts The penalty disappears once you turn 65 or become disabled. After that, non-medical withdrawals are still taxed as ordinary income but carry no penalty. Death of the account holder also waives the penalty for beneficiaries.
Form 5498-SA: Your Contribution Record
Form 5498-SA reports your total HSA contributions for the year, any rollover amounts, and the fair market value of the account. It arrives later than most tax documents (often not until May) because it covers contributions made all the way through the filing deadline.17Internal Revenue Service. Instructions for Forms 1099-SA and 5498-SA You do not need it to file your return. Your custodian sends the same information to the IRS. Keep the form to verify the contribution amount you reported on Form 8889 matches what the custodian sent in.
For 2026, the maximum you can contribute is $4,400 for self-only HDHP coverage or $8,750 for family coverage.18Internal Revenue Service. Revenue Procedure 2025-19 If you’re 55 or older, you can add another $1,000 as a catch-up contribution. Your plan also has to meet the 2026 HDHP thresholds for you to contribute at all.19Internal Revenue Service. IRS Notice 2026-05
How to Get Your Forms
Kaiser makes Forms 1095-B, 1095-C, and 1099-SA available through the secure member portal at kp.org, usually in a tax documents section.1Kaiser Permanente. Understanding Your IRS 1095 Forms Digital copies tend to appear before the mailed versions. The IRS deadline for insurers and employers to provide these forms is January 31.2Internal Revenue Service. If you don’t have your form by mid-February, contact Kaiser Permanente Member Services and ask for a replacement. The deadline for filing individual returns for tax year 2025 is April 15, 2026, so you have some runway, but don’t wait.20Internal Revenue Service. IRS Opens 2026 Filing Season
Form 1095-A is never on Kaiser’s portal. You retrieve it from the Marketplace where you enrolled, either HealthCare.gov or your state exchange. Log into the same account you used to apply for coverage and look for the tax forms section.
Fixing Errors on Your Forms
If a Kaiser-issued form contains wrong information, such as incorrect coverage months on a 1095-B or an inaccurate distribution total on a 1099-SA, contact Kaiser Permanente Member Services. Kaiser will review the issue and, if confirmed, issue a corrected form marked accordingly. Use the corrected version when you prepare your return.
Errors on Form 1095-A take a different path. Only the Marketplace can fix it. Contact the exchange where you enrolled and ask them to correct the underlying enrollment data; the Marketplace then generates a corrected 1095-A. The process can take several weeks, so start early if you spot a problem.
Already filed and then received a corrected form that changes your tax liability? You’ll need to file an amended return using Form 1040-X. Filing with information you know is wrong creates more problems than waiting for the correction or filing an extension.
