Missing Refund? How to Initiate an IRS Refund Trace With Form 3911

An IRS refund trace is a formal request for the agency to investigate a refund that was issued but never reached you. You can start one through the Where’s My Refund tool at irs.gov/refunds, the automated line at 800-829-1954, by speaking with an agent at 800-829-1040, or by mailing Form 3911. The IRS won’t act until enough time has passed for the payment to reasonably arrive, and resolution takes anywhere from about six weeks to 120 days depending on whether your refund was a paper check or a direct deposit.

When You Can Ask for a Trace

The waiting window depends on how your refund was sent. For direct deposit on an e-filed return, the IRS processes most refunds within 21 days; if the deposit hasn’t arrived five days after that window closes, you can request a trace. A trace is also appropriate when Where’s My Refund shows the refund was issued but your bank confirms no deposit posted, or when a paper check you received was later lost, destroyed, or stolen.

Before you file anything, do two quick checks. Ask your bank whether a deposit was rejected or is pending. Confirm the address on file with the IRS matches where you live now. Rule Out an Offset First

Sometimes a refund isn’t missing. It was taken to pay a debt. The Treasury Offset Program intercepts federal payments to collect past-due obligations owed to federal and state agencies, including unpaid child support, defaulted student loans, and overdue taxes.3Bureau of the Fiscal Service. If a federal tax debt caused the offset, the IRS sends a CP49 notice explaining the amount applied.4Internal Revenue Service. How to Start the Trace

Form 3911 isn’t always required. Most individual filers (single, head of household, or married filing separately) have three faster options:

  • Where’s My Refund at irs.gov/refunds can initiate a trace directly.
  • The automated line at 800-829-1954 walks you through the process.
  • Calling 800-829-1040 puts you through to a representative who can start it.

Joint filers face one restriction: the automated tools are off-limits. If you filed married filing jointly, you either call 800-829-1040 and speak with a representative or mail Form 3911.6Internal Revenue Service. Filling Out Form 3911

  • The tax year and filing status from the return the refund belongs to
  • The exact refund amount as shown on that return
  • The date you filed
  • The refund issuance date, which you can find in Where’s My Refund

Section I captures your identifying information. Section II asks what went wrong. For a direct deposit that never landed, check the appropriate box and include the routing number, account number, and bank name. If the bank rejected the deposit, note the reason if you know it; a closed account or a transposed routing number entered during e-filing are common causes. For a paper check that was lost after arrival, or one you believe was stolen and cashed, provide any details you have about how the check was handled.

Getting the refund amount and issuance date exactly right matters more than anything else on the form. Small discrepancies can slow the investigation or cause the trace to be rejected.

Form 3911 goes to a specific IRS refund inquiry unit assigned by your state, not to a general processing address. The addresses appear on the Form 3911 instructions page.7Internal Revenue Service. Keep a copy and get proof of mailing, such as a certified mail receipt. If an IRS agent completed the form for you over the phone, they may mail it to you to sign before the trace is officially opened.

What Happens After You File

The IRS checks its records against Treasury’s payment history. What comes next depends on the refund type.

Paper Check, Never Cashed

If Treasury confirms the check was never cashed, the IRS cancels it and issues a replacement. Paper Check, Already Cashed

If Treasury records show the check was cashed, the Bureau of the Fiscal Service mails you a claim package, usually including a copy of the cashed check with the endorsement on the back. You return the completed package, and BFS reviews the claim and the signature to decide whether to issue a replacement. That review can take up to six weeks.6Internal Revenue Service. Direct Deposit

Electronic traces can take longer than paper. IRS procedures allow up to 120 days for the full process.8Internal Revenue Service. 21.4.2 Refund Trace and Limited Payability If the trace confirms the bank rejected the deposit and returned the funds to the IRS, you’ll get a notice with next steps. The IRS typically reissues the refund as a paper check rather than trying another electronic transfer.

Wrong Bank Account

If you entered an incorrect routing or account number and the deposit went to someone else’s account, contact the financial institution directly first and ask that the funds be returned.9Internal Revenue Service. Refund Inquiries 18

If the bank rejected the deposit and sent it back to the IRS, you’ll receive a notice. If the bank accepted it into the other account and five calendar days pass without resolution, file Form 3911 so the IRS can contact the bank on your behalf. One hard truth: if the funds are gone or the bank refuses to return them, the IRS cannot force the bank’s hand. At that point it becomes a civil matter between you and the bank or the account holder who received your money.9Internal Revenue Service. The Three-Year Deadline

Refund claims expire. You generally have three years from the date you filed the return, or two years from the date you paid the tax, whichever is later, to claim a credit or refund. The IRS calls this the Refund Statute Expiration Date.10Internal Revenue Service. A few exceptions push the deadline out: filing from a combat zone or contingency operation, a presidentially declared disaster, and bad debt or worthless security losses, which get a seven-year window.10Internal Revenue Service. Trace or Identity Theft?

These solve different problems. A trace is for a refund the IRS issued that never reached you. An identity theft affidavit, filed on Form 14039, is for when someone used your Social Security number to file a fraudulent return. Signs that point toward identity theft include an e-filed return rejected because one was already filed under your SSN, notices about income from an employer you never worked for, or bills for a year you didn’t file.
