The mailing address for IRS Form 433-D is the one printed on the most recent IRS notice or letter that told you to complete the form. There is no single nationwide address. The notice will list either a specific IRS office, a service center, or a dedicated fax number tied to the employee or unit handling your case, and that is where your signed form needs to go. If you no longer have the notice, call the IRS at the number on any recent correspondence, or fall back to the general service center for your state (listed below).
Why the Notice Address Matters
Form 433-D finalizes an installment agreement. It locks in your monthly payment amount, due date, and, for a Direct Debit Installment Agreement, the bank account the IRS will pull from each month.1Internal Revenue Service. Form 433-D Installment Agreement You typically receive it after speaking with an Automated Collection System representative or a Revenue Officer who has already reviewed your account and negotiated terms with you.
That is why the notice address matters. The person or unit that worked out your terms is expecting your paperwork. Sending it to a general processing site instead of the collections office assigned to your case can delay your agreement by weeks while it gets rerouted internally. Any dedicated fax number on the notice is almost always faster than mail, because collections units process faxed agreements directly.
If You Don’t Have the Notice
Call the IRS first. A representative can look up your account and give you the exact address or fax number for the employee handling your case. Use the phone number on any prior IRS letter you still have.
If you need a mailing address and cannot reach the assigned office, the IRS publishes general service center addresses for individual balance-due payments by state of residence.2Internal Revenue Service. Where to Send Your Individual Tax Account Balance Due Payments These are payment addresses, not collections addresses, so use them only as a fallback:

Other Ways to Submit Form 433-D
Fax
If your IRS notice includes a fax number, faxing is usually the fastest option. Collections units often prefer it. Keep the fax confirmation page as your proof of transmission and the date sent.
Certified Mail

IRS Document Upload Tool
The IRS Document Upload Tool accepts documents submitted electronically in response to an IRS notice or letter. You’ll need the access code from your notice or the notice number itself, along with your Social Security number or employer identification number.3Internal Revenue Service. IRS Document Upload Tool This is a good option when you don’t have a fax machine but want to move faster than paper mail.
In Person at a Taxpayer Assistance Center

Collection Information Statement Financial statements are generally required when your balance exceeds $50,000, though the IRS can ask for one at any balance level if it wants an updated picture of your finances.
A Note on Which Form You Actually Need
Form 433-D is not the request for a payment plan; it’s the document that finalizes one after the IRS has approved terms. The initial request is Form 9465.6Internal Revenue Service. About Form 9465, Installment Agreement Request If you owe $50,000 or less and haven’t yet contacted the IRS, you can often set up an agreement entirely through the Online Payment Agreement tool without needing Form 433-D at all, and the setup fee is lower.7Internal Revenue Service. Online Payment Agreement Application Form 433-D comes into play when the IRS specifically asks you to complete it, usually after a call with a collections representative.
After You Submit
The IRS generally responds to installment agreement paperwork within 30 days, though filing season can stretch that timeline.8Internal Revenue Service. Topic No. 202, Tax Payment Options You’ll receive a letter that either accepts the agreement (confirming your payment amount, due date, and first debit date) or rejects it and explains why. A rejection often comes with a request for more financial information.
Keep paying while you wait. Interest accrues daily on unpaid tax until the full balance is satisfied, so making voluntary payments during the review period reduces what you owe and shows good faith.9Internal Revenue Service. Interest Once the agreement is in effect, the failure-to-pay penalty drops from 0.5% per month to 0.25%, provided you filed your return on time.10Internal Revenue Service. Failure to Pay Penalty
If you know you can’t make a scheduled payment, call the IRS before the due date. Form 433-D itself directs taxpayers to contact the IRS immediately if a payment can’t be made, and a proactive call is often what keeps an agreement from being terminated.1Internal Revenue Service.
