How to Report Your CSA 1099-R on Your Tax Return

To report your CSA 1099-R on your tax return, put the gross distribution from Box 1 on Form 1040 Line 5a, the taxable amount from Box 2a on Line 5b, and the federal tax withheld from Box 4 on Line 25a.1Internal Revenue Service. Instructions for Form 1040 For most federal retirees that is the whole job, because the Office of Personnel Management has already done the taxable-portion math and reported the result in Box 2a. A few situations change what you enter or add an extra step: a checked Box 2b, certain distribution codes in Box 7, the public safety officer health insurance exclusion, and state withholding. Each is covered below.

The Boxes That Actually Go on Your Return

The CSA 1099-R is a busy form, but only a handful of boxes end up on your 1040.

  • Box 1 (Gross Distribution): the full amount OPM paid you during the year, before any deductions. Goes on Line 5a.2Internal Revenue Service. Instructions for Forms 1099-R and 5498
  • Box 2a (Taxable Amount): the portion of Box 1 that counts as taxable income. OPM calculates this using the Simplified Method and reports the result here. Goes on Line 5b.2Internal Revenue Service. Instructions for Forms 1099-R and 5498
  • Box 2b (Taxable Amount Not Determined / Total Distribution): a checkbox. If “Taxable Amount Not Determined” is checked, OPM did not calculate the taxable portion and you must figure it yourself. Otherwise Box 2a is reliable.2Internal Revenue Service. Instructions for Forms 1099-R and 5498
  • Box 4 (Federal Income Tax Withheld): what OPM already sent to the IRS. Goes on Line 25a, where it credits against your final tax bill.2Internal Revenue Service. Instructions for Forms 1099-R and 5498
  • Box 7 (Distribution Code): a letter or number that tells the IRS what kind of payment this is. It can change where on Form 1040 the income goes.2Internal Revenue Service. Instructions for Forms 1099-R and 5498

Box 5 (Employee Contributions or Insurance Premiums) and Box 9b (Total Employee Contributions) are informational. They are already reflected in the gap between Box 1 and Box 2a, and you do not enter them anywhere on Form 1040.3Internal Revenue Service. Instructions for Forms 1099-R and 5498

If your annuity is fully taxable — meaning you have already recovered all of your after-tax contributions and Box 1 equals Box 2a — you can skip Line 5a and put the full amount on Line 5b.

What to Do If Box 2b Is Checked “Not Determined”

If OPM checked “Taxable Amount Not Determined” in Box 2b, you cannot rely on Box 2a. You have to calculate the taxable portion yourself using the Simplified Method worksheet in IRS Publication 721.4Internal Revenue Service. A separate calculation applies to a small group of retirees whose annuity started before July 2, 1986, or who elected the General Rule during the window that closed on November 18, 1996. Those retirees use the actuarial tables in Publication 939 instead. Anyone whose annuity started after November 18, 1996, cannot use the General Rule.6Internal Revenue Service. How the Box 7 Distribution Code Changes Your Entries

Most federal retirees see Code 7 (normal distribution) in Box 7, and the standard Line 5a / Line 5b entries apply. Three other codes come up often enough to know.

Code 3 (Disability)

If you retired under a disability provision, your annuity is taxed as wages — not pension income — until you reach minimum retirement age. Report those payments on Line 1 of Form 1040 (the wages line), not Line 5.4Internal Revenue Service. Code 4 (Survivor Annuity)

Code 4 appears when annuity payments go to a surviving spouse or other beneficiary after the retiree’s death. These payments are generally fully taxable. The exception: if the original retiree had not finished recovering their after-tax contributions, the survivor continues the monthly exclusion the retiree was using, and Box 2a will be less than Box 1 accordingly.

Code G (Direct Rollover)

Code G means the funds went straight from your federal retirement plan to another eligible retirement plan, such as a traditional IRA. The distribution is not taxable in the year it happened, but you still report it: enter the total on Line 5a and zero on Line 5b.

Federal Withholding Goes on Line 25a

The Box 4 amount lands on Line 25a of Form 1040, where it gets combined with any other federal withholding you had during the year. That total is credited against your final tax. Over-withheld, you get a refund; under-withheld, you owe the difference. You can adjust your OPM withholding at any time through Services Online or by submitting Form W-4P.

The Public Safety Officer Exclusion You Have to Claim Yourself

If you retired as a law enforcement officer, firefighter, chaplain, or member of a rescue squad or ambulance crew, you may be able to exclude up to $3,000 per year of your annuity for health or long-term care insurance premiums.8Office of the Law Revision Counsel. 26 USC 402 – Taxability of Beneficiary of Employees Trust The premiums can cover you, your spouse, or your dependents, and they must be paid from the retirement plan maintained by the employer you retired from as a public safety officer.9Internal Revenue Service. OPM does not apply this exclusion for you. Box 2a on your CSA 1099-R does not reflect it. To claim it, reduce the taxable amount on Line 5b by the qualifying premiums (up to the $3,000 cap). You cannot also deduct those same premiums as an itemized medical expense.9Internal Revenue Service. Publication 575, Pension and Annuity Income

State Income Tax Withholding

If you elected state withholding through OPM, the amount withheld appears on the CSA 1099-R along with the state’s identification number. Both go on your state return. How your annuity itself is taxed at the state level varies: some states fully exempt CSRS and FERS income, some offer age- or income-based partial exemptions, and some tax it the same as any other income. Check your state’s current rules — they change more often than most retirees expect, and moving to a new state can change the answer entirely.

If the Form Is Wrong or Hasn’t Arrived

If a figure on your CSA 1099-R doesn’t match your records, contact OPM Retirement Services and request a correction. OPM will issue a corrected version identified as CSA 1099-R-C.10U.S. Office of Personnel Management. Get Your 1099-R Tax Form

If the filing deadline is close and no correction has arrived, you have two practical options. For a small discrepancy, file with the figures you know are correct and submit an amended return on Form 1040-X after the corrected form comes in.11Internal Revenue Service.
