Getting Your W-2 After Leaving CVS
Your ColleagueZone access is shut off once you leave, but the MyHR CVS portal keeps limited functionality for former employees to view pay stubs and tax documents. Log in with your old CVS Colleague ID and the password you set while employed. Forgot the password? Use the reset link and recover it through your security questions or the personal email address on file.
If MyHR won’t let you in, the backup route is eprintview.com/cvs. The credentials there work differently. Your user ID is your seven-digit employee number. The initial password may be your last four Social Security digits followed by your two-digit birth month and four-digit birth year. The system may also ask for an employer PIN. These credentials can change without notice.
When both portals fail, call the CVS Health HR Service Center at 888-694-7287 and ask them to send you a copy. Have your Colleague ID, full name, the tax year you need, and your current mailing address ready.
Getting Your W-2 as a Current Employee
Current employees retrieve the W-2 through ColleagueZone. Sign in with your CVS user ID and password. The login typically requires a second verification step, such as a code sent to your phone or email. On the main dashboard, look for a tile labeled “My Pay” or “Employee Self-Service,” open the payroll section, and select “Tax Forms” or “W-2.” Pick the tax year, download the PDF, and save it somewhere secure.
If you’ve never opted in for electronic delivery, check the “Tax Forms” section for a consent option. The IRS requires employers to get your affirmative consent before delivering a W-2 electronically instead of mailing a paper copy, so without that consent, yours goes out by mail.
When Your W-2 Should Be Available
Federal law requires employers to furnish W-2 forms by January 31 of the year following the tax year. When that date falls on a weekend or holiday, the deadline shifts to the next business day. If Your W-2 Never Arrives
Start with the address on file. Log in to MyHR and confirm your mailing address is current. If it’s right and you still don’t have the form by mid-February, call the CVS Health HR Service Center at 888-694-7287 and ask them to resend it or make the electronic version available.
Still nothing by the end of February? Call the IRS at 800-829-1040. Have your name, address, Social Security number, dates of employment, and CVS’s name, address, and phone number ready. Form 4852 is a last resort, not a shortcut. The IRS expects you to have tried getting the real W-2 from your employer and to have waited past the February deadline before using it.3Internal Revenue Service. If those estimates turn out to be wrong once the real W-2 shows up, you’ll need to file an amended return.
If Something on Your W-2 Is Wrong
Don’t file with figures you know are wrong, and don’t try to fix the form yourself. The employer issues a corrected form, called a W-2c.4Internal Revenue Service. Specifics move it faster.
A quick sanity check before you flag anything: compare Box 1 against your final pay stub’s year-to-date taxable wages. They should match or come close. A big gap in either direction usually traces to a mid-year benefits change that wasn’t processed correctly or a bonus taxed at a different rate.
If CVS doesn’t correct the error by the end of February, call the IRS at 800-829-1040 to initiate a formal W-2 complaint, or visit an IRS Taxpayer Assistance Center in person. If a W-2c Arrives After You Filed

You generally have three years from the date you filed your original return, or two years from the date you paid the tax, whichever is later, to file the amendment and claim any refund you’re owed.6Internal Revenue Service. If the correction means you underpaid and you delay, the IRS can add an accuracy-related penalty of 20% of the underpayment, plus interest that compounds daily until the balance is cleared.7Internal Revenue Service. Protecting the Social Security Number on Your W-2
The form carries your full Social Security number, which makes it a target. If you suspect your W-2 was intercepted in the mail or your information was otherwise compromised, move quickly.
- Report the theft at IdentityTheft.gov, which walks you through fraud alerts, reports, and account security.
- Contact the Social Security Administration if you believe someone filed a fraudulent W-2 using your Social Security number.
- Request an Identity Protection PIN from the IRS through your online account at IRS.gov. The IP PIN is a six-digit number that prevents anyone else from filing a return using your Social Security number, and you can request one even if you haven’t been a victim of identity theft.8Internal Revenue Service. The simplest preventive step, if you’re still at CVS, is opting in for electronic delivery through ColleagueZone. A PDF in a secured portal can’t be pulled out of your mailbox.
