CVS tax forms come from three different places depending on your relationship with the company. Current employees pull their W-2 from Workday through the myHR platform. Former employees use the AlumniZone portal at alumnizone.cvs.com. Independent contractors and vendors retrieve 1099 forms through the CVS Health Accounts Payable Vendor Portal. The same routes apply whether you worked for CVS Pharmacy, Aetna, CVS Caremark, or MinuteClinic.
W-2 or 1099: Which One You Should Expect
If CVS employed you directly and withheld taxes from your paychecks, you get a W-2 reporting wages and federal, state, and local withholding.1Internal Revenue Service. About Form W-2, Wage and Tax Statement The subsidiary on your paycheck doesn’t change that.
If you provided services as a contractor or vendor, you get a 1099 instead. For 2026, the reporting threshold for Form 1099-NEC moved from $600 to $2,000. If CVS paid you $2,000 or more for nonemployee services during the calendar year, you’ll receive a 1099-NEC.2Internal Revenue Service. 2026 Publication 1099 Other variants such as 1099-MISC may apply depending on the payment type.
One catch worth flagging: if CVS paid you less than $2,000 as a contractor, you probably won’t get a 1099-NEC. You still have to report the income. The threshold governs whether CVS must send a form, not whether the money is taxable.
Getting Your W-2 as a Current CVS Employee
Active employees view pay information and tax documents in Workday, the company’s HR and payroll system.3CVS Health. CVS Health Benefits Overview To see your W-2 digitally before the paper copy is mailed, you need to have consented to electronic delivery inside the system. Without that consent, a paper copy goes out by mail.
Logging in takes your 7-digit Employee or Contractor ID and your password. Lost either one? Your manager or the CVS Health HR Service Center can reset access. Test your login before late January. That’s when everyone else remembers they need their W-2 and support lines back up.
Getting Your W-2 as a Former CVS Employee
After you leave CVS, your W-2 lives on the AlumniZone portal at alumnizone.cvs.com.4CVS Health. AlumniZone Homepage The portal typically keeps several prior tax years available. You’ll need your 7-digit CVS Colleague ID to sign in.
A paper copy also gets mailed. Federal law requires employers to furnish W-2s by January 31 of the following year.5Office of the Law Revision Counsel. 26 USC 6051 – Receipts for Employees When January 31 lands on a weekend, the deadline shifts to the next business day. That mailed copy goes to whatever address CVS had on file the day you left. If you’ve moved, contact the CVS Health HR Service Center to update the address before year-end. Chasing a W-2 that went to an old apartment in February is avoidable.
Getting a 1099 as a Vendor or Contractor
Independent contractors and vendors retrieve 1099-NEC and other 1099-series forms through the CVS Health Accounts Payable Vendor Portal.7CVS Health. Payers must furnish 1099-NEC forms to recipients by January 31.8Internal Revenue Service. If Your Form Doesn’t Show Up
The IRS recommends a specific order. First, contact the employer or payer. For W-2 issues, call the CVS Health HR Service Center. For 1099 issues, contact Accounts Payable. If the form still hasn’t arrived by the end of February, call the IRS at 800-829-1040. You’ll need your name, address, Social Security number, phone number, dates of employment, and CVS’s name and address. The IRS will contact CVS on your behalf.10Internal Revenue Service. If the filing deadline is closing in and the W-2 still hasn’t arrived, you can file using Form 4852 as a substitute, estimating wages and withholding from your final pay stub.11Internal Revenue Service. If the actual W-2 arrives later with different numbers, you’ll file an amended return.
Fixing Errors on a CVS W-2 or 1099
An incorrect Social Security number, misspelled name, or wrong wage figure on a W-2 requires CVS to issue a Form W-2c, showing both the original and corrected amounts.12Internal Revenue Service. For 1099 discrepancies, contact Accounts Payable with your Vendor ID or TIN.
1099 errors follow the same pattern: CVS reissues the form with the correct figures. Don’t file with numbers you already know are wrong. Wait for the corrected version if the deadline allows.
If you already filed and a corrected form arrives afterward, you may need to file Form 1040-X. You generally have three years from when you filed the original return, or two years from when you paid the tax, whichever is later.13Internal Revenue Service.
