Completed Form 1040-X Example: Part I, Part II, and Attachments

A completed Form 1040-X example is easiest to follow as a single worked scenario: the form uses three columns to reconcile a return you already filed with the corrected version, so Column A holds your original figures, Column B holds the dollar change on each line, and Column C holds the corrected figures. Below is a full walk-through using one taxpayer’s numbers, from the header down through the explanation of changes.

The Scenario

Sarah is a single filer who submitted her 2025 Form 1040 in February 2026. Her original return showed:

  • Adjusted gross income: $50,000
  • Standard deduction: $15,000
  • Taxable income: $35,000
  • Tax: $3,962
  • Federal income tax withheld: $4,800
  • Refund claimed: $838

In April 2026, Sarah finds her Form 1098-E showing $2,500 of student loan interest paid during 2025. She never claimed the deduction. Student loan interest is an adjustment to income on Schedule 1, so it lowers AGI directly.

Before touching the 1040-X, she reworks the return with the deduction included. Corrected AGI is $47,500. The standard deduction is unchanged, so corrected taxable income is $32,500. Running that through the 2025 tax tables, her corrected tax is $3,662, a $300 reduction. Withholding stays at $4,800, so her total overpayment becomes $1,138 rather than $838. The extra $300 is what the amendment will recover.

Every number in Column C needs to be settled before you open the 1040-X. The form reconciles; it doesn’t calculate.

The Header

At the top of Form 1040-X, Sarah enters her name, Social Security number, and address, and writes 2025 as the calendar year being amended. She checks the box for “Single” filing status. If her filing status were changing, she would check the corrected status here and indicate the original status in the space provided. On a joint amendment, both spouses have to sign.1IRS. Part I: The Three Columns, Line by Line

Part I is where the reconciliation happens. Column A is what you originally reported, Column B is the increase or decrease, and Column C is the corrected amount.2Internal Revenue Service. Here is how each affected line on Sarah’s 1040-X reads.

Line 1 — Adjusted Gross Income

  • Column A: $50,000 (from Line 11 of the original 1040)
  • Column B: ($2,500), a decrease from adding the student loan interest deduction
  • Column C: $47,500

Line 2 — Itemized or Standard Deduction

  • Column A: $15,000
  • Column B: $0 (Sarah took the standard deduction; that isn’t changing)
  • Column C: $15,000

Line 5 — Taxable Income

  • Column A: $35,000
  • Column B: ($2,500)
  • Column C: $32,500

Line 6 — Tax

  • Column A: $3,962
  • Column B: ($300), the tax savings from the lower taxable income
  • Column C: $3,662

Lines 7 through 10 cover nonrefundable credits and other taxes. Sarah has none, so those stay at zero across all three columns. Line 11, total tax, tracks Line 6: $3,962 in Column A, $3,662 in Column C.

Payments and Refund Lines

The payments section doesn’t use the three-column format. Sarah reports her $4,800 of withholding on Line 14 and leaves Lines 12, 13, and 15 blank because she had no estimated payments, no amount paid with the original return, and no refundable credits.

Line 18 asks for the overpayment already shown on the original return. Sarah enters $838, the refund she has already received. Line 22, the amount to be refunded on the amendment, comes out to $300. That is the check or deposit she is claiming.

Part II: Explanation of Changes

Every 1040-X has to include a clear written explanation of what changed and why.3IRS. Instructions for Form 1040-X (Rev. December 2025) Vague wording like “correcting deductions” invites a follow-up letter. Sarah writes:

“Amending to claim the student loan interest deduction of $2,500 on Schedule 1, Line 21, which was inadvertently omitted from the original return filed February 15, 2026. This reduces AGI on Line 1 from $50,000 to $47,500 and reduces tax on Line 6 from $3,962 to $3,662. Corrected Schedule 1 and Form 1098-E are attached.”

The explanation names the specific line, states the dollar impact, and lists the attached documents. That is what a reviewer needs to close out the amendment quickly.

What to Attach

For a paper-filed 1040-X, you have to attach a completed, updated Form 1040 or 1040-SR reflecting all the changes, along with any revised schedules.3IRS. Instructions for Form 1040-X (Rev. December 2025) For Sarah, that is a corrected Schedule 1 showing the $2,500 student loan interest deduction and a copy of her Form 1098-E.

Corrected W-2s, W-2Cs, or 1099-Rs that show tax withheld get attached to the front of the form. Other supporting documents such as K-1s or receipts go behind the 1040-X. Label any attached returns “Copy Only — Do Not Process” at the top so the IRS does not treat them as new filings.

Filing the Amendment

Form 1040-X can be e-filed through tax software for the current year and the two prior tax periods.4Internal Revenue Service. About Form 1040-X, Amended U.S. Individual Income Tax Return For 2026, that covers 2023, 2024, and 2025. Older years have to be paper-filed.

E-filing has one refund-related consequence worth knowing about. If your amendment produces a refund, an e-filed 1040-X lets you choose direct deposit. A paper-filed amendment always produces a paper check.2Internal Revenue Service. Instructions for Form 1040-X The refund from the amendment arrives separately from any refund you received on the original return, so Sarah’s $300 will come as its own deposit.

If you mail the form, the IRS address depends on your state and is listed in the current 1040-X instructions. It is not necessarily the same address you used for your original return.

When a 1040-X Isn’t the Right Form

A few situations look like they call for an amendment but don’t. The IRS automatically corrects simple math errors, wrong tax-table entries, and mismatched Social Security numbers; you will get a notice and no amendment is needed. December 2025) And if you discover a mistake before the original filing deadline, wait for the first return to process rather than sending a second original.

State Return

A federal amendment almost always requires an amended state return as well. Nearly all states with an income tax require you to report the federal change, notification deadlines run from about 30 to 180 days after the federal change becomes final, and each state uses its own form. Check your state revenue agency’s site for the specific deadline and form so a clean federal amendment doesn’t leave a state-level penalty behind it.
