Box 12 Code DD on W-2: What It Covers and Why It’s Not Taxable

Code DD on a W-2 sits in Box 12 and shows the total cost of your employer-sponsored health coverage for the year, combining what your employer paid and what came out of your paycheck. It is reported for information only. What the Dollar Figure Represents

The number next to Code DD is the aggregate cost of your group health plan for the calendar year. Aggregate means it rolls together every dollar your employer contributed toward premiums and every dollar you contributed, whether your share came out pre-tax or after-tax.2Internal Revenue Service. For family coverage, the total commonly reaches five figures. Seeing $20,000 or more in Box 12 with Code DD is not unusual and does not mean something went wrong on your W-2. Most employees underestimate what health coverage costs because the employer’s share never lands in a paycheck.

Why Code DD Is Not Taxable

The Code DD figure does not appear in Box 1 (wages), Box 3 (Social Security wages), or Box 5 (Medicare wages). It sits in Box 12 precisely because it is outside your taxable compensation. Two provisions of the tax code keep it there. Code DD is not subject to federal income tax withholding or FICA. A large number here means your health plan is expensive. It does not mean you owe anything extra. It also has no effect on your adjusted gross income, so it does not change your eligibility for the premium tax credit on the health insurance marketplace.

What the Amount Includes

Not every health-related benefit ends up in the Code DD total. The main categories that get folded in:

  • Major medical coverage under your employer-sponsored group health plan, including medical, surgical, and hospital benefits.
  • Dental and vision coverage when bundled into the same group health plan as your medical coverage. Standalone dental or vision plans that you can decline separately or that require a separate premium are generally left out under current transition relief.5Internal Revenue Service.   • Employee assistance programs, under the same COBRA-premium test as wellness programs.

Why Some W-2s Have No Code DD

A blank Code DD does not mean your employer made a mistake, and it does not mean your coverage has a tax problem. The IRS has kept broad transition relief in place since 2012, with no expiration announced as of early 2026.1Internal Revenue Service. The IRS has committed to at least six months of advance notice before changing any of these exemptions. If you work for a smaller company or get coverage through a union plan, an empty Code DD is likely the reason.

How to Enter Code DD on Your Return

Type the Code DD amount into Box 12 of your tax software exactly as it appears on your W-2. The software will recognize the code and will not add the figure to any income line on Form 1040. Enter it accurately anyway. The IRS runs automated matching on W-2 data, and a missing or mistyped Code DD can generate correspondence or slow processing even though the amount is non-taxable.

If Code DD Looks Wrong

If the amount seems significantly off, say $2,000 for a plan you know costs $18,000, contact your employer’s payroll or HR department. Employers are responsible for issuing a corrected W-2 on Form W-2c when a previously filed W-2 contains an error. Because Code DD is informational, an incorrect amount will not cause you to overpay or underpay tax. The correction matters more for your employer’s compliance record than for your return. Still, if you notice the error, flag it. It may point to a broader payroll issue that could affect taxable items on future W-2s.
